As we are eager to constantly improve the quality of The Embassy, we have invited you to participate in a survey that will help us learn more about whether The Embassy has fulfilled your expectations. Completing the survey will take you about 5 minutes.
Instructions
- Multiple choice questions: you can select the response that best suits your opinion. The questionnaire goes further automatically. Be aware, if you click on "enter" without selecting a number (1-5), the default answer is 1.
- Open questions and explanations: you can press "enter" or click on "continue" to go further. You can go back to previous questions by using the arrows in the bottom right of the screen.
Your Privacy
We value your privacy. Our Privacy statement is in place. There are small exceptions due to the nature of the evaluation. Your input is anonymised at all times. That means you cannot ask us to remove it afterwards. However, you can quit the questionnaire at any time. If you exit the survey before submission, your reaction is not stored. In addition, the questionnaire allows you to go back in your session. This allows you to adapt your feedback. Your participation is entirely voluntary and you may exit the survey anytime you wish. The anonymous data will be used to evaluate The Embassy and for research purposes by the projects that developed The Embassy. The anonymous data may be shared for research purposes with similar projects, the European Commission and authorized auditors.
Data storage
We start recording your data from the moment you choose to participate. We will download and delete the data from Typeform every two weeks. Downloaded data we store offline on local servers of Amsterdam UMC. We save data for 15 years. Because the data is anonymous, we will not be able to retrieve and delete data after the data has been stored.
